Submit a Claim
Please provide your details and the details of the deceased member below. Our team will review your claim promptly. Outside business hours (7:00 AM – 7:00 PM), submit the form below and we will attend to your request when our office reopens.
What you need to do
Follow these steps to submit your claim successfully.

Obtain the required death documents
Get a Notice of Death (DHA-1663 A) and a Medical Certificate of Cause of Death (DHA-1663 B).

Verify the notice of death
Ensure the death notice includes the left thumb print of the deceased as well as the informant's thumb print.

Contact our office
Reach us via call, WhatsApp or email and provide us with the claim details along with the notice of death.

Choose a burial society
Contact the burial society whose services you wish to use and provide us with their details once you have decided on a provider. We can provide a list of providers upon request.

We take it from there
We will contact the burial society to confirm details and may send a representative on the day.
